2009 Band Trip

Parents and Students, Please print the attachments for complete information regarding the STEP
ONE: DEADLINE – 12/01/08 (Send agreement to
Kaleidoscope Adventures’ office) Each
participant MUST COMPLETE a hardcopy Individual Payment Agreement and
mail, fax or scan/email the page with your name/address & contact
information to our office. Very Important – this ensures smooth
payment posting. See attachment. STEP
TWO: DEADLINE – 12/12/08 (Create PayPal account and make first payment - $50.00
non-refundable) All
participants MUST go online to the Kaleidoscope Adventures website and complete
their registration for the trip, and make their first payment to be guaranteed
a place on the trip. NO PAYMENTS CAN BE CREDITED toward individual
students and chaperones, until EACH HAS CREATED A “PAYPAL” ACCOUNT AND SENT IN
THEIR Individual Payment Agreement (Step One above). Go to www.kaleidoscopeadventures.com Click the Blue Button on
the lower left side of the homepage screen. Follow the instructions. STEP
THREE: DEADLINE – 01/15/09 (Send Permission form
and make second payment - $173.00) All
participants, including chaperones, must complete the Permission to Travel
form and send it to Kaleidoscope Adventures’ office. See attachment –
2 pages. Make your second payment no later than this date. STEP
FOUR: DEADLINE – 02/11/09 (Rooming list is due
from Mr. Henderson) No action
required from parents or students STEP FIVE: DEADLINE – 02/20/09 (Make final payment - $172.00 estimated; based on final
numbers of participants) ** Less
than 35 days prior to departure, all payments are non-refundable under any
circumstances. You may opt to purchase travel insurance to protect your
trip investment. See Mr. Henderson for a travel insurance brochure and
